Telehealth Consent

This is a general telehealth informed-consent notice and may be supplemented by state-specific language. Please read it carefully before using the Service.

By using Vitale Health (the “Service”), you consent to receive medical services via telehealth, including asynchronous (store-and-forward) telehealth in which a licensed clinician reviews the health information you submit and makes clinical decisions without a real-time visit. This consent explains the nature of telehealth, its benefits and limitations, and your rights.

What telehealth is

Telehealth is the delivery of health care services using electronic communications and information technology rather than an in-person visit. Through the Service, you provide a medical history and intake information that a licensed clinician reviews to determine whether treatment is appropriate and, where appropriate, to issue a prescription.

Benefits

  • Convenient access to a licensed clinician without travel or scheduling an office visit;
  • Timely review of your health information;
  • Confidential handling of your information consistent with applicable privacy laws.

Limitations and risks

You understand and accept that:

  • No in-person physical examination will be performed;
  • The clinician relies on the information you provide, so inaccurate or incomplete information may lead to an incorrect assessment or to treatment being declined;
  • Not every condition is appropriate for telehealth evaluation, and the clinician may determine that you should be seen in person;
  • As with any medical service, there are potential risks, including that a condition may not be diagnosed or that a treatment may not produce the expected result;
  • Technical failures or security issues could, in rare cases, affect the transmission of your information.

Not for emergencies

Telehealth through the Service is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room immediately.

Your rights

Your participation in telehealth is voluntary. You have the right to decline telehealth and to seek in-person care instead. The information disclosed during your use of the Service is protected under applicable federal and state privacy laws, as described in our Privacy Policy. You have the right to ask questions about your care and to request copies of your records.

Consent and withdrawal

By using the Service, you acknowledge that you have read and understood this consent, that you have had the opportunity to ask questions, and that you consent to receive care via telehealth. You may withdraw your consent at any time, except to the extent that action has already been taken in reliance on it, by contacting support@vitalehealth.shop.
